Job summary

Vitability Health is transforming remote care delivery by enabling medical teams to provide safe, effective remote care that improves outcomes and lowers costs. As part of our care team, you will perform routine nursing tasks, educate patients, and help develop new processes for RPM programs.

Responsibilities include vital signs, point-of-care testing, maintaining records, and delivering motivational coaching to drive positive health outcomes.

Qualifications

• Bilingual a plus.
• Experience with Medicare patients.
• Interest in leadership growth opportunities.
• Technically savvy; comfortable using iPads for charting.
• Willing to operate in a new clinical program and help develop processes.
• Strong verbal and written communication skills.
• Valid driver's license with a clean driving record.
• Ability to lift/carry up to 20 pounds.
• Willing to drive to patient homes in NYC.
• COVID-19 vaccination required.

Responsibilities

• Vital signs.
• Point-of-care tests (glucometer, etc.).
• Maintain patient medical records.
• Documentation consistent with state regulation.
• Use motivational communication techniques to drive health behavior change.
• Educate patients on managing acute or chronic conditions to maintain health and avoid ER visits.
• Impact longitudinal patient engagement in RPM programs.
• Provide preventive health and disease management education and coaching.
