Job summary

Vivo Care, Inc. is seeking a Care Navigator to build trust with patients and drive positive health behavior changes. You will triage vital signs via a Remote Care Platform, collaborate with the care team, and develop patient-centered action plans to improve health and well-being.

The role emphasizes monitoring RPM data, providing preventive education, and supporting Medicare patients through telephonic care management. Remote work is supported from a clinical team perspective.

Qualifications

• RN/LPN/LVN license in a compact state is required.
• Experience with phone triage and remote care delivery preferred.
• Experience with Medicare patients or care coordination is a plus.
• Ability to work MST time zone (8:00a–5:00pm) is required.
• Spanish-speaking ability is a plus.

Responsibilities

• Triages vital signs and uses data to promote health behavior changes.
• Conducts monthly wellness calls and defines patient goals with the care team.
• Educates and coaches patients on action plans for better health.
• Monitors physiological markers and supports RPM/CCM workflows.
• Manages patient messaging, alerts, and escalation pathways.
