Happy Vitals PC is seeking a Remote CNA/MA to support RPM and CCM programs. This backend clinical role emphasizes accuracy, accountability, and remote collaboration under RN supervision using clearly defined clinical protocols.

You will review patient data, document interactions in the EHR, and help with care coordination and outreach while maintaining HIPAA compliance and high-quality, technology-enabled care.

Qualifications

• Active CNA or CMA/RMA required
• 1–2 years healthcare experience (RPM/CCM/primary care/home health preferred)
• Experience with electronic health record (EHR) systems
• Strong written and verbal communication skills
• High attention to detail and ability to manage multiple priorities
• Bilingual is a plus

Responsibilities

• Review daily RPM data and identify abnormal readings
• Escalate findings to supervising RN per workflow
• Document patient interactions and monitoring data in the EHR
• Ensure documentation supports RPM and CCM program requirements
• Maintain HIPAA compliance and patient confidentiality
• Conduct wellness check-ins and follow-ups with patients
• Provide reminders and basic care coordination support
• Document all patient communications clearly
• Follow standardized workflows and escalation pathways
• Meet productivity and quality benchmarks; participate in quality improvement
• Assist onboarding of new team members as needed
