Job summary

Medical Director Services PC is seeking compassionate RN or LPN to join our Remote Patient Monitoring (RPM) team in a fully remote role. You will monitor patients' vital signs, review biometric data, and escalate clinically significant findings to providers to improve outcomes.

You'll work independently from a quiet home office with reliable internet, documenting interactions in the EMR and collaborating with clinicians to ensure timely follow-up and Medicare RPM compliance.

Qualifications

• Active RN or LPN license in good standing.
• Strong assessment and critical thinking skills.
• Excellent verbal and written communication.
• Experience with EMR and remote monitoring tech.
• Ability to work independently with multiple patients.

Responsibilities

• Monitor daily patient vital signs through RPM platform.
• Review blood pressure, pulse, oxygen saturation, weight, blood glucose, temperature, and other biometric data.
• Identify abnormal trends and elevate clinically significant findings to the appropriate provider.
• Communicate with patients by phone to assess symptoms, provide education, and encourage adherence to care plans.
• Document all patient interactions and clinical interventions accurately in the EMR.
• Collaborate with nurse practitioners, physicians, and the clinical team to ensure timely follow-up.
• Meet Medicare RPM documentation requirements while delivering high-quality patient care.
