Job summary

KangarooHealth is a nationwide remote care provider delivering RPM, CCM, and PCM programs. As a Care Access Specialist, you’ll be the patient’s welcome contact, enrolling them in RPM/CCM, answering questions, and guiding onboarding.

Qualifications

• Active LPN or LVN license with multi-state compact privileges.
• 3+ years of clinical experience in a hospital, family practice, or geriatric setting.
• 3+ years of EMR documentation experience; ability to learn new platforms quickly.
• 1+ year of remote work experience with patient phone engagement.
• Strong understanding of chronic conditions and their management (HTN, DM, CHF, COPD).
• Familiarity with Medicare, CCM/RPM program requirements, and CMS billing codes.
• Technical fluency with G-Suite, Microsoft Office, Hubspot, Slack, Zoom, Epic, ECW, Athena, and similar platforms.
• Reliable high-speed internet, dedicated computer, and a private, HIPAA-compliant home workspace.
• Available to work 40 hours per week aligned with client clinic hours 9AM–6PM local time.

Responsibilities

• Enrollment: Make outbound calls to eligible RPM and CCM patients; explain programs; address skepticism; secure verbal consent and complete enrollment; meet daily enrollment targets with quality.
• Initial CCM Care Call: Conduct first CCM care call; cover care plan review, education on chronic conditions, and patient goals; provide health coaching; identify urgent needs and escalate; document activities in EMR per billing protocol; apply CCM billing codes.
