Job summary

Seamless Assist is seeking a full-time, remote clinical pharmacist with cardiology expertise to support value-based care programs. The focus is GDMT optimization, titration, and close collaboration with RN Care Managers and cardiology providers to close therapy gaps.

This is a non-dispensing, telephonic role embedded in the care team. You will document recommendations in the care management platform/EMR, educate patients, and adhere to Central Time business hours. Texas license required.

Qualifications

• PharmD with Texas license required at time of hire.
• 3–5+ years of clinical pharmacy experience with a cardiology focus.
• Experience with GDMT and HFrEF titration.
• Working knowledge of ACC/AHA guidelines.
• Telephonic patient communication experience.
• Able to work Central Time business hours.
• Excellent written and verbal English.

Responsibilities

• Review cardiology medication profiles and optimize GDMT per ACC/AHA guidelines.
• Lead GDMT assessment and titration with providers and nursing staff.
• Conduct telephonic medication reviews and patient education.
• Collaborate with RN Care Managers and cardiology providers to align medication plans.
