Job summary

United States Digital Space LLC is seeking a Clinical Pharmacist with a strong cardiology focus for a remote, full-time contract role dedicated to optimizing guideline-directed medical therapy and titration of cardiac medications within a nurse-led, interdisciplinary care model.

You will perform telephonic medication reviews, collaborate with RN Care Managers and cardiology providers, document in the EMR, ensure multi-state licensure and active Texas license, and support value-based care

Qualifications

• 3–5 years of clinical cardiology-focused experience.
• PharmD with active Texas pharmacist license; multi-state licensure required.

Responsibilities

• Medication Review & GDMT Optimization in cardiology populations.
• Telephonic patient education and engagement on cardiac medications.
• Collaborate with RN Care Managers and cardiology providers; document recommendations in EMR.
• Support quality and value-based care initiatives and reduce adverse events.
