Job summary

Symba Center is seeking a CalAIM Referral Coordinator to oversee the full lifecycle of referrals across CalAIM programs. You will manage intake, maintain trackers, and coordinate with internal teams and external partners to optimize program revenue.

Responsibilities include processing referrals, verifying eligibility, and ensuring HIPAA-compliant documentation within required timeframes. Strong communication and organizational skills are essential for success in a remote/hybrid setup.

Qualifications

• High school diploma required.
• Minimum 1 year of experience in healthcare administration, social services, or case management, ideally within Medi-Cal/CalAIM.
• Familiarity with CalAIM populations and Community Supports is preferred.

Responsibilities

• Receive, review, and track CalAIM referrals from health plans, providers, hospitals, and community partners.
• Triage eligibility verification for ECM and Community Supports per policy.
• Coordinate warm handoffs between referral sources, members, and internal care teams.
• Maintain and update referral tracking systems to ensure data integrity and timely submission.
• Serve as primary point of contact for external partners regarding referral status and coordination.
• Ensure HIPAA-compliant, accurate documentation submitted within turnaround times.
• Maintain referral records in EHR and care management systems; support audits and QA initiatives.
