Job summary

Center for Community Resources in Williamsport, PA is seeking a Blended Case Manager to support adolescents, children, and adults with serious mental illness or emotional disturbances. The role includes coordinating services, advocacy, documentation, and timely reporting.

The position requires a Bachelor’s degree in Human Services or related field and at least one year in MH case management, with on-call duties and potential evening/weekend hours.

Qualifications

• Bachelor’s degree in Human Services or related field.
• One year experience in MH Case Management or in the mental health field.
• Knowledge of case management principles; computer literate; familiarity with the Recovery Model; knowledge of local resources (Clinton and Lycoming County).

Responsibilities

• Provide blended case management services to individuals with mental health diagnoses.
• Complete reports and documentation in a timely fashion.
• Participate in an on-call rotation to ensure 24-hour coverage during holidays, weekends, and daily hours outside regular schedule.
