Devoted Health is seeking a Bilingual Spanish/English Transitions of Care Advanced Practice Provider (APRNs or PAs) to lead post-discharge care and prevent readmissions in a fully remote Texas-based role. The position entails ~70% initial post-discharge visits and ~30% follow-up/urgent visits, with a focus on comprehensive care planning within a 30-day window.

Qualifikationen

• 5+ years of direct patient care managing geriatric patients with multiple chronic conditions.
• Strong clinical decision-making and bias to action in a virtual setting.
• Fluency in English and Spanish is required.
• Ability to work full-time 5 days a week, 8 hours daily (Mon–Fri 8:30–17:00 CST/PST/MST).

Aufgaben

• Perform comprehensive initial TOC visits and post-discharge assessments.
• Conduct acute/virtual urgent-care TOC visits and manage deteriorations.
• Oversee the 30-day clinical trajectory and adjust plans as needed.
• Participate in interdisciplinary TOC rounds and quality-improvement projects.
• Contribute to staff education and EHR optimization, and support program growth.
