Alignment Health is seeking a Remote RN Case Manager SNP in the United States to coordinate care for members with complex and chronic needs. The nurse will engage with members by telephone, perform comprehensive assessments, and collaborate with PCPs and the care team to close gaps in care.

Must hold an active California RN license (Non-Compact) and be willing to obtain additional licenses as needed; 8:00am–5:00pm Pacific. Salary range reflected with market-based compensation.

Formación

• RN licensure active and unrestricted (California Non-Compact)
• Minimum 2 years' clinical experience and 1 year case management experience
• AS in Nursing required; BSN preferred
• Strong knowledge of community resources, chronic care management, and home health

Responsabilidades

• Coordinates care as a resource for the member, family and physician.
• Facilitates referrals/authorizations within the benefit structure.
• Completes comprehensive assessments within scope of practice.
• Collaborates with the member, PCP, and care team to implement a care plan.
• Interfaces with PCPs, Hospitalists, NPs and specialists on care plans.
• Provides education and self-management support.
• Assists in problem solving with providers, claims or service issues.
• Works with contracted providers to assure care coordination.
• Maintains confidentiality of PHI in compliance with laws.
