Job summary

Alignment Health is seeking a Remote RN Case Manager SNP to coordinate care for members with complex and chronic conditions. Must hold an active California RN license (Non-Compact) and be willing to obtain licenses in other states as needed.

The role requires 8:00am–5:00pm Pacific Time, collaboration with a multidisciplinary team, and a focus on closing gaps in care while maintaining PHI confidentiality. Alignment Health is an Equal Opportunity Employer.

Qualifications

• Require: 2+ years clinical experience and 1+ year case management experience.
• Preferred: Health Plan experience.
• RN licensure in California (Non-Compact) required; willingness to obtain LVN/RN licensure in other states as needed (reimbursed).

Responsibilities

• Coordinates care for the member, family, and physician.
• Facilitates referrals/authorizations within the benefit structure.
• Completes comprehensive assessments within scope of practice.
• Collaborates with the member, the PCP, and care team to implement a plan of care.
• Interfases with PCPs, Hospitalists, NPs, and specialists on treatment plans.
• Provides education and self-management support.
• Assists in problem solving with providers, claims or service issues.
• Works with delegated or contracted providers to assure coordination.
• Maintains confidentiality of all PHI.
