Job summary

HealthSnap is hiring bilingual LPNs to support chronic care management and remote patient monitoring. As a Care Navigator, you will work with the patient’s care team to empower patients to manage their chronic conditions and improve health outcomes.

The role runs Monday–Friday, 9:00am–5:30pm ET, with training on HealthSnap’s platform. Compact nursing license required; 3+ years in relevant settings; strong Spanish and communication skills; EHR proficiency and HIPAA compliance are essential for

Qualifications

• Current, valid Multistate/Compact LPN/LVN license in good standing.
• At least 3+ years in primary care, cardiology, internal medicine, home care or CCM/RPM.
• Excellent communication and Spanish language ability; team player.

Responsibilities

• Provide patient support via phone consultations for care management and RPM.
• Educate patients on conditions and empower lifestyle changes per care plans.
• Maintain accurate patient records and document interactions per protocol.
• Address barriers to care and propose practical adherence improvements.
• Communicate clearly with patients and care teams per approved workflows.
• Participate in training, team meetings, and quality initiatives.
• Respond to remotely transmitted patient data per partner workflows.
