Job summary

HealthSnap is seeking a Care Navigator with an active Michigan LPN license to support patients in chronic care management and remote patient monitoring programs.

You will educate patients on conditions like hypertension and diabetes, maintain patient records, and collaborate with care teams to empower patients to manage their health. The role runs Monday–Friday, 9:00 am–5:30 pm ET, in Michigan.

Qualifications

• Current, valid Michigan LPN license in good standing.
• Additional state licenses may be required and reimbursed by HealthSnap.
• 3+ years of experience in primary care, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring.

Responsibilities

• Complete phone consultations with patients in care management and/or remote monitoring programs to provide support and education about chronic conditions.
• Educate patients about health conditions and empower them with lifestyle and behavior strategies to manage chronic conditions.
• Maintain accurate, up-to-date patient records and ensure all interactions and care plans are documented per protocol.
• Address patient concerns and barriers to care, and work to improve adherence and outcomes.
• Provide clear, compassionate communication to patients and follow workflows for communicating needs to providers.
• Participate in training sessions, team meetings, and quality improvement initiatives.
• Respond to remotely transmitted patient data (e.g., BP, glucose, weight, pulse oximetry) per partner workflows.
