Job summary

DDRC of Lakewood, CO is seeking a full-time Bilingual Service Coordinator/Case Manager for Early Intervention (Birth-3) to coordinate services for young children and their families.

Most work is remote with some travel around Jefferson County. Typical schedule is Mon-Thu 8:00 AM–5:30 PM. Compensation ranges from $25.25 to $27.78 per hour, with medical, dental, vision, 401(k) and generous leave benefits.

Qualifications

• Fluency in English and Spanish (verbal and written).
• Five years of case management experience if no degree.
• Experience or familiarity with Early Intervention (EI) or IDD systems preferred.

Responsibilities

• Coordinate EI services from eligibility through transition for birth-3 children.
• Lead IFSP meetings and ensure timely, family-centered services.
• Explain EI processes and connect families with community resources.
• Plan and coordinate smooth transitions to preschool options and related timelines.
• Maintain documentation, consent, and confidentiality in compliance with regulations.
