Job summary

DDRC, the Developmental Disabilities Resource Center in Lakewood, CO, seeks a full-time Bilingual Service Coordinator/Case Manager for Early Intervention (Birth-3). This remote-first role travels around Jefferson County and acts as the primary contact for families navigating EI services.

Responsibilities include leading IFSP meetings, coordinating transitions from EI to preschool, and ensuring services are delivered on time with family-centered care.

Qualifications

• Bachelor's degree in early childhood education, social work, or related field, or equivalent experience.
• Five years of relevant case management experience if no degree.
• Demonstrated verbal and written fluency in both English and Spanish languages.

Responsibilities

• Coordinate EI services from eligibility through transition for birth-3 children.
• Lead IFSP meetings and ensure services are delivered timely and family-centered.
• Explain EI process to families and connect them with community resources.
• Maintain accurate documentation, consent and confidentiality per regulations.
• Participate in team meetings, trainings and professional development.
