Job summary

Service Access & Management, Inc. is seeking a Service Coordinator – Early Intervention to support children under age 3 with developmental delays. You will coordinate services, conduct meetings, and ensure continuity of care across families and providers.

Starting rate is $20.00/hour, with potential for growth based on experience and qualifications. The role is based in Luzerne/Wyoming counties in Pennsylvania, with community-based work and some remote flexibility.

Qualifications

• Bachelor’s degree in Early Childhood Education, Psychology, or related field and at least 1 year of human services experience.
• OR an associate’s degree or 60 college credits in Early Childhood Education with 3 years of relevant experience.
• Strong customer service and time management skills required.

Responsibilities

• Request and complete service authorizations in a timely manner.
• Conduct Family Service Plan meetings and ensure OCDEL standard compliance.
• Coordinate services and referrals to support each child’s development.
• Participate in Multidisciplinary Team meetings for collaborative planning.
• Develop and update transition plans to maintain continuity of care.
• Maintain positive relationships with families, providers, and community partners.
• Document all services and communications accurately and on time.
