Responsibilities

• Serve as a primary point of contact for families in Early Intervention.
• Help families understand and access services and resources.
• Coordinate services between families, providers, therapists, and professionals.
• Maintain regular communication with families regarding services and progress.
• Assist with scheduling and coordinating evaluations, meetings, and services.
• Follow up with providers regarding service delivery and outstanding matters.
• Maintain accurate and organized case documentation.
• Complete required reports, notes, and other program documentation.
• Monitor cases and ensure required follow-ups are completed in a timely manner.
• Assist families with accessing appropriate community and program resources.
• Participate in meetings and collaborate with other members of the child's service team.
• Maintain confidentiality and follow applicable program requirements and procedures.
