Job summary

Developmental Disabilities Resource Center (DDRC) of Lakewood, CO seeks a full-time Bilingual Service Coordinator/Case Manager for Early Intervention (Birth-3) to support families and coordinate EI services.

The role emphasizes English/Spanish fluency, strong communication, and collaboration with families, providers, and community resources in a remote-first environment with some travel around Jefferson County.

Qualifications

• Bachelor's degree in early childhood education, social work, or related field, or equivalent experience.
• Five years of relevant case management experience if no degree.
• Demonstrated verbal and written fluency in both English and Spanish.

Responsibilities

• Coordinate EI services from eligibility through transition for birth-3.
• Lead IFSP meetings and ensure timely service delivery.
• Serve as the primary contact for families and connect to community resources.
• Document accurately and maintain confidentiality per state and federal regulations.
• Collaborate with families, providers, and partners in a family-centered approach.
