Job summary

Scriptology is seeking a highly skilled Ambulatory Care Clinical Pharmacist fluent in Spanish to join our team as a PRN remote clinician. You will provide comprehensive medication management in an ambulatory setting, optimize therapy, and support patients navigating their medication journeys in collaboration with prescribers.

You will conduct med reviews, reconcile meds, document assessments, and participate in quality initiatives to improve outcomes.

Qualifications

• Pharm.D. degree from an accredited school of pharmacy.
• Active pharmacist license; CA/NY/TX preferred.
• NPI required.
• BCACP or ASHP residency preferred.
• Spanish fluency required (healthcare level).

Responsibilities

• Close clinical gaps to improve CMS Star ratings and HEDIS measures.
• Conduct comprehensive medication reviews for safety and efficacy.
• Maintain accurate patient records including medication profiles.
• Document assessments, recommendations, and follow-up plans per regulations.
• Identify and resolve medication-related problems with evidence-based changes.
• Provide medication reconciliation to support safe transitions of care.
• Serve as a trusted advisor to patients and prescribers; answer questions.
• Collaborate with physicians, nurses, and allied staff for coordinated care.
• Participate in quality improvement initiatives and stay current with guidelines.
