Job summary

Desort is seeking a Bilingual Clinical Pharmacist to join its virtual care team. The role emphasizes direct patient education, medication management, and collaboration with physicians to improve outcomes.

This position offers primarily remote work with required in-person training. You will work with patients and healthcare providers to identify medication concerns, support individualized treatment plans, and enhance adherence across diverse populations.

Qualifications

• Minimum 2 years of clinical pharmacy experience or equivalent combination of education and experience.
• Current, unrestricted pharmacist license in the applicable state(s).
• Fluent English and Spanish communication skills required.

Responsibilities

• Review medication histories and evaluate the appropriateness of prescribed therapies.
• Communicate with patients and physicians regarding medication use and adherence.
• Identify barriers to adherence and develop practical solutions.
• Educate patients on safe and effective use of medications.
• Collaborate with physicians when therapy review is needed.
• Support clinical teams with medication-related expertise and disease-management initiatives.
• Provide compassionate, culturally responsive service to English- and Spanish-speaking patients.
