Qualifications

Responsibilities

• Lead and coach Care Managers to ensure high-quality patient care.
• Monitor queues, assignments, documentation and care-plan tasks for timeliness.
• Coordinate with clinics and internal leadership to ensure smooth operations.
• Provide direct patient support when needed and balance workload.
• Escalate clinical, compliance or staffing issues to leadership when appropriate.
