Job summary

Evergreen Nephrology is seeking a Certified Medical Assistant to support Chronic Care Management for patients with CKD, hypertension, diabetes, and related conditions. You will conduct outreach, create and reinforce care plans, and ensure documentation accuracy across CCM and EHR systems.

This fully remote role requires 8:00am–5:00pm Pacific time, bilingual English/Spanish, and 1–3 years of healthcare experience.

Qualifications

• Certified Medical Assistant with active certification.
• Bilingual Spanish/English required.
• 1–3 years healthcare experience.
• Experience in chronic disease or nephrology preferred.
• Strong clinical assessment, communication, and documentation skills.
• Able to work across EHR and CCM platforms.

Responsibilities

• Support CCM patient enrollment and onboarding.
• Outreach to enrolled CCM patients weekly via text and phone.
• Document patient interactions in CCM and EHR systems.
• Maintain HIPAA and PHI compliance in all activities.
• Coordinate care transitions and referrals.
• Collaborate with remote clinical and operational teams.
