Job summary

This role supports proactive care coordination for patients with complex chronic conditions, including CKD. You will conduct high-volume outreach, enroll and onboard patients into CCM programs, and coordinate referrals and follow-ups using EHR and CCM platforms daily, while maintaining audit-ready, HIPAA-compliant documentation.

Your Role:

• Support proactive care coordination for patients with complex chronic conditions including CKD.
• Conduct high-volume text and phone outreach to engage patients and reinforce care plans.
• Maintain audit-ready documentation and ensure HIPAA compliance.
