Job summary

Oak Street Health - Rocket City is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy within our primary care clinics. The role collaborates with care teams and a psychiatric consultant to coordinate behavioral health care.

Requires master's degree in mental health counseling or related field, LICSW/LPC/LMFT license, and experience with screening, assessment, and treatment planning for common mental health disorders.

Qualifications

• Master's degree in mental health counseling or related field from an accredited school.
• Highest clinical license (LICSW/LPC/LMFT) or local equivalent.
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health conditions.
• Familiarity with brief, structured interventions (e.g., Motivational Interviewing, CBT).

Responsibilities

• Collaborate with primary care teams to identify patients for referral to the Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and facilitate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including telehealth handoffs and care plans.
