Job summary

Oak Street Health - Rocket City seeks a Behavioral Health Specialist to provide care management and short-term psychotherapy for patients with behavioral health needs. The role collaborates with care teams and a psychiatric consultant to coordinate services and referrals.

Requires LICSW, LPC or LMFT and a Master's in a mental health field; US work authorization. Some remote work days may be available based on program metrics, with a typical 40-hour work week and competitive benefits.

Qualifications

• Master's Degree in mental health counseling/healing arts from an accredited school.
• Highest clinical level of license (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health conditions.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to refer patients to the Oak Street Behavioral Health Program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short‑term counseling and evidence‑based treatments.
• Coordinate care with psychiatric consultants, including warm handoffs for telehealth visits.
• Maintain accurate documentation of patient encounters in EMR.
• Participate in regional and organizational efforts to advance the program.
