Oak Street Health - Rocket City is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy for patients with behavioral health needs. You will collaborate with primary care teams and a psychiatric consultant to develop care plans and coordinate referrals.

Responsibilities include screening, assessment, and brief interventions, with potential remote work days based on performance and program metrics.

Qualifications

• Master's degree in mental health counseling/healing arts from an accredited school.
• Highest clinical license or local equivalent.
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health/substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Cognitive Behavioral).
• US work authorization.

Responsabilités

• Collaborate with primary care teams to identify patients for referral to Oak Street Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and coordinate with Oak Street's psychiatric consultant and external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with the psychiatric consultant, including warm handoffs for telehealth visits and care plan collaboration.
• Develop relationships with primary care teams and communicate changes in behavioral health care plans.
• Accurately document patient encounters and counseling sessions in Oak Street's EMR.
• Participate in regional and organizational efforts to advance the Behavioral Health Program.
• Possibly cover multiple clinics depending on maturity and panel sizes.
