Job summary

Urban Health Plan is seeking a Screening Specialist to address behavioral health and social service referrals from providers. Under supervision, you will assess patient needs and coordinate internal/external referrals, escalating crises to Licensed Social Workers.

You will also provide concrete services for needs such as housing or food insecurity. This remote position runs Monday–Friday 8am–5pm with 1–2 late nights per week, and onsite support as needed for meetings and trainings.

Qualifications

• Bachelor's degree in Psychology, Social Work, or related field required.
• 2+ years of case management experience.
• Knowledge of social services programs and resources.
• Ability to multi-task and manage several requests via phone/computers.
• Excellent communication skills.
• Dedicated to supporting patients in the community.

Responsibilities

• Review referral box and assess patient needs using EHR.
• Provide BH referrals and arrange internal/external appointments.
• Assist with housing and food insecurity needs.
• Assist with IEP educational issues and scheduling of appointments.
• Escalate high-risk or complex cases to Social Work team.
• Document all contacts and interventions in the system.
