Independence Blue Cross is seeking a Case Manager - Behavioral Health (Triage) for fully remote work within the Tri-State Area (PA/NJ/DE). The role provides telephone triage, crisis intervention, and care navigation to at-risk members with mental health and/or substance use concerns.

You will assess needs, connect members with appropriate providers, support care plans, and advocate within the health care team.

Qualifications

• Active PA RN license required.
• 3–5 years post-licensure in behavioral health clinical work.
• Experience in Utilization Review or Case Management preferred.
• Knowledge of DSM-5 and ASAM.
• Strong MS Office skills.

Responsabilités

• Provides telephone triage and care navigation for at-risk members.
• Engages with members and families to assess needs and promote access to services.
• Deescalates callers and provides brief counseling in crisis situations.
• Matches members with behavioral health providers based on needs.
• Supports care coordination and advancement of care plans.
