Job summary

Elevance Health seeks a Social Work Case Manager to support members in managing chronic conditions through coordinated care and resource navigation. This role offers hybrid work and opportunities for impact across behavioral health and community resources.

Requires an MSW and at least 3 years of case management experience in healthcare, plus an unrestricted LMSW or LCSW license. Fluency in Spanish is a plus, with a strong emphasis on collaboration and technical proficiency.

Qualifications

• MS (at a minimum) in Social Work with a minimum of 3 years in case management in a healthcare setting.
• Current unrestricted LMSW or LCSW license in applicable state(s) is required.

Responsibilities

• Utilizes available community, government, and client resources to address participant limits or support interventions for chronic condition management.
• Manages behavioral and psychosocial needs to improve clinical and financial outcomes and delivers social work interventions.
• Assists members to effectively utilize resources to meet health needs and develop capabilities.
• Evaluates members' ability to self-manage and locate resources when limitations are identified.
• Provides guidance to members seeking solutions to social, cultural, or financial problems affecting healthcare management.
• Evaluates members' health self-management strengths and develops strategies to support healthcare needs and implement plans.
