Job summary

Ascend Healthcare is seeking a remote Case Manager to coordinate behavioral health services and address social determinants of health. You will collaborate with clients, families, and providers to ensure access to appropriate services and improve health outcomes.

The role emphasizes outreach, care coordination, documentation, and compassionate patient advocacy. A bilingual English/Spanish background and a healthcare setting experience are preferred; remote work with occasional in-person needs

Qualifications

• Bachelor’s degree in social work, psychology, counseling, human services, public health, or 3 years of behavioral health experience in integrated care.
• Knowledge of community resources and social service systems.
• Strong organizational, communication, and documentation skills.
• Ability to establish rapport with diverse populations and maintain professional boundaries.
• Proficiency with electronic health records and Microsoft Office applications.

Responsibilities

• Maintain accurate, timely, and complete documentation in accordance with organizational, regulatory, and payer requirements.
• Complete evidence-based screening tools, assessments, service plans, progress notes, and discharge summaries within established timeframes.
• Conduct comprehensive case management assessments to identify client strengths, needs, barriers, and goals.
• Assist with safety planning and resource coordination, recognizing and responding appropriately to crisis situations within the scope of the position.
• Provide outreach and engagement services to patients who are at risk of disengaging from treatment.
• Coordinate medical, behavioral health, substance use, housing, employment, educational, transportation, and social support services.
• Support patients in addressing social determinants of health that impact treatment outcomes.
• Participate in staff meetings, training, and professional development opportunities.
• Support organizational initiatives focused on quality, patient experience, and health outcomes.
• Perform other duties as assigned consistent with the responsibilities of the position.
