Job summary

Molina Healthcare is seeking a TX-licensed Behavioral Health clinician to join our remote care management team. The role supports Medicaid members with behavioral health needs via telephonic coordination, with occasional in-person visits within Texas. Strong IT skills and prior MCO case management experience are preferred.

Location is Texas; hours are M–F, 8 AM–5 PM in your time zone. Experience with routine clinical software and MS Office is required to support documentation and collaboration

Qualifications

• ;Licensed BH clinician with active Texas license per state requirements; minimum 2 years healthcare/behavioral health experience; valid driver's license and reliable transportation; experience with care coordination and clinical platforms; strong verbal and written communication; proficiency in Microsoft Office.

Responsibilities

• Completes comprehensive behavioral health assessments per timelines and determines eligibility for care coordination.
• Develops and implements care plans with member and care network to address needs and goals.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness, documents interventions and outcomes, and suggests changes.
• Maintains ongoing member caseload with regular outreach.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and ICT collaboration.
• Uses motivational interviewing and Molina guidelines to support change.
• Assesses barriers to care and provides coordination assistance as needed.
• May provide consultation to peers as needed.
• Travel 25-40% may be required.
