Responsibilities

• Develops, assesses, and facilitates complex care management activities for mental and behavioral health needs.

• Educates members and families on procedures, treatment options, referrals, and benefits.
• Monitors care plans and member status to adjust treatment as needed.
• Collects and maintains member information to ensure regulatory compliance.
