Job summary

Devoted Health is seeking a Remote Advanced Practice Provider (NP or PA) to support chronic disease management for nationwide members. This full-time role requires weekend coverage with 3x12-hour shifts, or 4x10s, including Saturdays and Sundays, 8am–8:30pm EST.

You will preside centralized renewals, manage results, and deliver virtual care via digital platforms. The role emphasizes cardiometabolic care, telehealth workflows, and collaboration with PCPs and care teams to improve patient

Qualifications

• APP with three or more years in outpatient clinical practice, with hypertension and lipid management focus.
• Experience in chronic disease management and primary prevention of cardiovascular disease.

Responsibilities

• Conduct virtual visits focusing on chronic health conditions and medication management.
• Manage centralized refill inbox and prescription renewals.
• Collaborate with cross-functional care team to coordinate patient care and outcomes.
• Utilize home-grown EHR for visits and asynchronous prescription refills.
