Job summary

Devoted Health seeks an experienced Advanced Practice Provider to join its EDC hub for chronic disease management. You will deliver virtual visits, manage centralized prescription renewals, and coordinate with PCPs and care teams across digital and traditional platforms.

Ideal candidates have hypertension and lipid management experience, 2+ years in primary care or subspecialties, and will obtain licensure in required states within 90 days of hire.

Qualifications

• APP with three or more years in outpatient clinical practice, ideally hypertension, hyperlipidemia, and ASCVD prevention.
• Minimum of 2 years in primary care or subspecialty with heavy focus on hypertension and lipid management.

Responsibilities

• Conduct focused assessments for chronic conditions and perform virtual visits (40–60%).
• Develop diagnoses and individualized treatment plans including med management and lifestyle changes.
• Manage prescription refill inbox per established protocols (40–60% of clinical effort).
• Participate in centralized results management and triage of labs and diagnostics.
• Utilize digital and AI-assisted tools to improve prescribing accuracy and patient communication.
