Job summary

Qualifications

• Must have degree in Physical Therapy; Doctor of Physical Therapy (DPT) preferred.
• Physical Therapy License in the state(s) of employment.
• Maintains Basic Life Support (BLS) certification for Healthcare Providers.
• Continuing education requirements as mandated by applicable state professional standards.

Responsibilities

• Provide age-appropriate patient care across the life span as per department policies.
• Interpret medical records and referrals; perform musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish evidence-based patient care plans including modalities, exercise programs, and education.
• Set functional goals with patient/caregiver; monitor outcomes and reassess as needed.
• Educate patients and caregivers; provide home programs and discharge instructions.
• Communicate with providers and staff; attend multi-disciplinary meetings as required.
• Complete documentation; ensure billing accuracy and adherence to policies.
• Maintain department cleanliness and proper equipment handling.
• Pursue professional development and participate in performance improvement initiatives.
• Manage caseload and coordinate coverage as needed across locations.
