Job summary

Open Sky Community Services is seeking a Relief Counselor to provide essential support to individuals facing mental health challenges, helping them live independently and reach their personal goals.

You will work with our relief team to implement person-centered plans, assist with daily living needs, and help clients access community resources while maintaining a safe and empowering environment. Training and supervision provided.

Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver’s License and acceptable driving record required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
