Job summary

Open Sky Community Services is seeking a Relief Counselor to support individuals facing mental health challenges, helping them live independently and pursue growth while ensuring daily needs are met in a safe, encouraging environment.

As part of our team, you will develop and implement person-centered support plans, help clients access community resources, document care, and promote skill development and meaningful relationships.

An additional hourly differential may apply in select programs.

Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver’s License and acceptable driving record.
• Reliability and willingness to learn; open to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
