Job summary

Open Sky Community Services is seeking a Relief Counselor to support individuals facing mental health challenges, helping them live independently and reach their goals. You will provide direct support, ensure daily living needs are met, and maintain a safe, Inspiring atmosphere within the program.

The position involves developing person-centered plans, assisting with community resources and transportation, and facilitating activities that promote growth, social connection, and skills.

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License and acceptable driving record, required.
• Reliability, willingness to learn, and being open to new opportunities.

Responsibilities

• Develop and implement individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
