You'll participate in developing person-centered plans, coordinate resources, and promote skill-building while maintaining documentation and accountability.

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License and acceptable driving record, required.
• Reliability, willingness to learn, and being open to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
