Open Sky Community Services is seeking a Relief Counselor to help individuals facing mental health challenges live independently and reach their goals. You’ll provide direct support, ensure daily living needs are met, and contribute to a safe, inspiring environment.

The role emphasizes person-centered plans, transportation assistance, and skill-building activities. A valid driver’s license and reliable schedule are required to join our trauma-informed, anti-racist, inclusive team.

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License and acceptable driving record, required.
• Reliability, willingness to learn, and being open to new opportunities.

Responsabilités

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
• The organization recognizes skills, location, training, and populations served in certain programs by offering an additional hourly differential.
