Job summary

Open Sky Community Services is seeking a Relief Counselor to support individuals facing mental health challenges and help them live independently. The role involves direct care, implementing person-centered plans, and ensuring daily living needs while maintaining a safe, encouraging environment.

The position offers a flexible schedule with an additional hourly differential in some programs. A High School Diploma or GED, a valid driver’s license, and reliability are required.

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License and acceptable driving record required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
