Role Overview

The Rehabilitation Coach, under the direction of the Coordinator of Community Day Services and supervision of the Rehabilitation Services Manager, ensures all vocational training opportunities are completed accurately and within timeline expectations; provides vocational instruction, training, and life skills. Works collaboratively with the individual and the Rehabilitation Team to identify the person's abilities, needs, preferences, desired outcomes, and other identified needs for developing and implementing individualized person‑centered plans and support high quality life skills developmental training and rehabilitation opportunities for persons participating in the Community Day Services program.

Responsibilities

• Assist the Production Team with documentation related to consumer trainee attendance, productivity, inventory and ordering supplies, completing time studies and other performance measurement studies.
• Provide on‑site job training and support in community employment and/or work trial settings in conjunction with the Director/Coordinator and CES Team.
• Teach and supervise work skills of consumers enrolled in Community Day Services, including implementing individualized person‑centered plans, monitoring and evaluating trainee progress, providing feedback, assessing work quality, and identifying/implementing individualized methods to facilitate progress related to productivity and earning capacity.
• Support individuals with developmental, intellectual, psychiatric and other disabilities as they make choices about their lives and play valuable roles in their communities.
• Encourage the people served to explore what they want out of life, develop and enhance life skills, and chart their own journeys, with the goal of assisting the person to lead a full and active life.
• Implement strategies to support the achievement of personal outcomes identified in the implementation strategy and help develop cognitive, emotional, and life functioning abilities of each consumer for rehabilitation.
• In coordination with the Production Manager, complete necessary time studies and performance measurement studies ensuring consistent implementation of time study procedures in day‑to‑day training intervention with consumers.
• Assist with materials handling, set up workstations, make reasonable accommodations and operate equipment necessary for contract completion.
• Provide rehabilitation services focused on the needs/desires of enrolled consumer trainees during periods when remunerative work is unavailable and assist with transportation needs.

Minimum Requirements

• High School Diploma or GED.
• At least 21 years of age.
• Experience working with persons with disabilities is preferred.
• Reliable transportation, valid driver’s license, and meet the organization’s insurability requirements.
• Employment is conditional upon completion of all background checks.
