• 2 years rehabilitation experience preferred.

Aufgaben

• Coordinate treatment plans with the interdisciplinary team.
• Establish tentative discharge plans and contingency plans.
• Support patient discharge planning and execution of the discharge experience.
• Monitor patient experience, quality, timeliness, and payor considerations.
• Maintain documentation and case management addendums; ensure regulatory compliance.

Ausbildung

Bachelor's degree or higher (health-related)

RN license (BSN preferred) or equivalent

CCM or ACM certification (required or within 2 years)
