Qualifications

• CCM® or ACM™ certification required or obtained within two years of placement.

Responsibilities

• Coordinate patient care from admission to discharge.
• Lead discharge planning and contingency plans with the interdisciplinary team.
• Monitor patient experience and care quality, including payor considerations.
• Attend ACT meetings to identify trends and drive process improvements.
• Ensure accurate case management documentation and addendums.
