Job summary

Encompass Health is seeking a Case Manager to connect exceptional care with meaningful patient outcomes in Naples, FL. You will manage resources, coordinate care from admission to post-discharge, and guide interdisciplinary plan-of-care decisions to support timely and quality patient journeys.

You will work with an interdisciplinary team to plan discharge, educate patients and families, and ensure accurate documentation while aligning with regulatory standards and payor requirements.

Qualifications

• Qualified to independently complete an assessment within scope of practice of discipline.
• Active license for the state in the relevant discipline.
• CCM® or ACM™ certification required or to be obtained within two years of entry into the position.
• CCM® or ACM™ certification must be obtained within two years of being placed in the Case Manager II position.

Responsibilities

• Work with interdisciplinary team to guide treatment plans based on patient needs and preferences.
• Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
• Participate in planning for and execution of patient discharge experience.
• Monitor patient experience: quality, timeliness, service appropriateness, payors, expectations.
• Facilitate weekly team conferences and coordinate treatment plan modifications.
• Complete case management addendums and required documentation.
• Maintain knowledge of regulations, policies, and department operations.
• Review case management reports and plan appropriate actions.
• Understand contract levels, exclusions, payor requirements, and recertification needs.
• Attend ACT meetings to identify trends and reduce ACTs.
• Meet with patient/family within 24 hours of admission; complete assessment within 48 hours.
