Qualifications

• Must be CCM® or ACM™ certified or eligible to obtain within two years of entry.
• Bachelor's degree in Nursing required; BSN preferred.
• Minimum two years rehabilitation or post-acute care experience preferred.

Responsibilities

• Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
• Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
• Participate in planning for and execution of patient discharge experience.
• Monitor patient experience, including quality, timeliness, and payer communications.
• Facilitate weekly team conferences and coordinate treatment plan modifications.
• Complete case management addendums and all required documentation.
