Qualifications

• Must be licensed to independently complete assessments within the scope of practice.
• Active license for the relevant discipline in the state of practice.
• CCM® or ACM™ certification required or obtained within two years.

Responsibilities

• Coordinate patient care from admission to post-discharge with the interdisciplinary team.
• Develop and monitor treatment plans based on patient needs and goals.
• Complete case management documentation and addenda accurately and timely.
• Lead discharge planning activities and communicate with payors and post-discharge providers.
