Qualifikationen

• Certification in Case Management or Rehab Nursing preferred.

• Proficiency with Meditech or similar EHR mentioned.

Aufgaben

• Coordinate management of care and ensure optimum utilization of resources and adherence to CMS guidelines.
• Complete Individual Plan of Care (IPoC) per CMS guidelines and care coordination.
• Schedule family conferences and communicate progress to patients and caregivers.
• Coordinate discharge planning including home health, DME, and post-acute services.
• Collaborate with nursing, therapy and ancillary staff to optimize patient care.
