Job summary

Lifepoint Rehabilitation at Hannibal Regional Hospital in Hannibal, MO is seeking a Case Manager - Rehabilitation to coordinate patient care, assess needs, develop and implement care plans, and ensure smooth discharge planning in a fast-paced inpatient setting.

You will collaborate with nursing, therapy and ancillary staff, track CMS guidelines and IPoC documentation, participate in weekly conferences, and identify opportunities to improve quality while controlling costs.

Qualifications

• Certification in Case Management or Rehabilitation Nursing preferred.
• 2+ years social work or case management in inpatient setting preferred.
• Excellent English communication; additional languages preferred.
• Basic computer skills: Excel, Word, Outlook, PowerPoint.
• Strong organization and time management; ability to work independently.
• Team player and ability to maintain confidentiality.

Responsibilities

• Coordinate management of care and ensure optimal resource use and compliance.
• Provide ongoing assessment, care planning, plan implementation and evaluation.
• Enhance patient management and satisfaction to promote continuity of care.
• Coordinate discharge planning including home health, equipment, therapies, and services.
• Lead or participate in conferences and communicate with caregivers and teams.
• Identify trends to improve quality and cost effectiveness; present to leadership.
• Perform intake assessments within 24–72 hours of admission.
• Document CMS/IPoC updates and payer communications.
